Supporting Evidence Form
Add a goal to your NDIS plan and complete the form. The NDIS will assess if an ILO fits and may fund the Exploration and Design Stage
Stage 1: Exploration and Design
Plan where and how you want to live, who you’ll live with, and the support you’ll need. Use this to create an NDIS Service Proposal.
Service Proposal Submission
Submit your plan to the NDIS to request ILO funding for your chosen supports.
Stage 2: Transition
Move into your ILO at your own pace, creating a flexible plan for the process
Monitor and Review
Update NDIS with changes or new costs during reassessments using a Change of Situation form and updated Service Proposal
