Why the School Leavers Transition Program?
Everyone is at the same stage

This is the only Co-Hive program where every single participant has just finished school. That shared experience matters more than you might think.
Connection happens quickly here.
A routine when you need one most

Leaving school means losing a lot of built-in structure.
We fill that gap with a weekly schedule that keeps participants busy, confident and looking forward to the week ahead.
Skills for everyday life

Getting around, managing money, using technology, communicating confidently.
We work on the practical stuff that makes a real difference to how independently someone can live.
Finding out what comes next

Each week includes tasters of other Co-Hive programs, plus introductions to TAFE, university and work pathways.
There's no pressure, just plenty of room to explore.
Support for families too

When your child finishes school, a lot changes for your family as well.
NDIS reviews, new funding, different services. Our team has been through it with many families and we're here to help you navigate it.
Friendships that last

Past school leavers have formed some genuinely close friendships through this program, particularly over December.
For a lot of them, it's opened up their world in ways nobody quite expected.

Frequently Asked Questions
What support is available for families during this transition?
Quite a lot. Our team can help with understanding your NDIS options, navigating plan reviews, applying for further education the following year and connecting you with the right people. Please reach out anytime.
Can participants connect with Co-Hive before the program starts?
Yes! Once your child has had their intake meeting and been confirmed as suitable, they're welcome to join our social meetups or independent getaways in the lead-up to October. It's a great way to make early connections. Click here to see upcoming events.
Has the program schedule been finalised?
Not yet, and that's okay. We'll release the full week-by-week layout closer to commencement. Each week will include tasters of different Co-Hive programs, pathway explorations and activities that build independence and community.
Do participants have to attend all ten weeks?
Yes. The program runs for the full ten weeks and consistent attendance matters, both for the participant's progress and for the cohort experience. The connections made early are a big part of what makes the program valuable.
My child is still attending school during Term 4. Can they still join?
Yes. Most participants will have finished school by the time the program begins, but some may still be in Term 4. What matters is that participants don't miss any scheduled transition activities. Please get in touch so we can plan together.
My child is still completing HSC exams in Term 4. Is that okay?
Absolutely. We encourage participants to start in Week 1 where possible, but a later start is fine. We'll work with you to make sure your child settles in well.
